Change Billing Cycle - Cycle 3 to 4

I hereby authorize Cookson Hills Electric Cooperative, Inc. to switch my account to Billing Cycle 4 (payment is due the 1st of each month). Upon switching billing cycles I understand that I will receive my regular monthly bill, which is due by the 15th of the month, and I will also receive an additional bill for approximately 15 days usage, which will be due by the 1st day of the following month. I understand that if I am currently enrolled in the Automatic Bank Draft program, the bank draft date for Cycle 4 will be processed around the 26th of each month prior to being due on the 1st of the next month. Please note that changes cannot be made 48 hours prior to the scheduled draft date. I understand that changing to a different billing cycle may take up to 45 days to process. I also understand that if I change billing cycles I must remain on the same billing cycle for at least 12 months before I can switch to a different billing cycle.
